About the Program

St. Francis Children's Center's Children's Long-Term Support (CLTS) Program serves children with Autism or other severe and long-term developmental, physical, emotional or sensory impairments between the ages of 3 through high school. The program is designed to provide support and service coordination for these children and their families in Milwaukee County.

Our service coordinators maintain regular supportive contacts with families through home visits and phone calls. They help facilitate services and collaborate with county staff as needed and ensure that program participants become connected to the government, medical, educational and community resources, programs and supports that will help to enrich their lives. Service coordinators are also responsible for the development of Individual Service Plans, which details goals and objectives for each child.

This program is just one more way the expertise of St. Francis Children's Center impacts the lives of children with special needs and their families.

For further information on this program, contact Program Manager Kendra Carter at (414) 351-8851 ext. 287.
