Children's Long-Term Support (CLTS) Program

St. Francis Children's Center's CLTS Program serves children with Autism or other severe and long-term developmental, physical, emotional or sensory impairments between the ages of 3 and 21. The program is designed to provide support and service coordination for these children and their families in Milwaukee, Ozaukee and Washington counties.

Our service coordinators maintain regular supportive contacts with families through home visits and phone calls. They help facilitate services with designated treatment providers, collaborate with state and county staff as needed and ensure that program participants become connected to the government, medical, educational and community resources, programs and supports that will help to enrich their lives. Service coordinators are also responsible for the development of Individual Service Plans, which details treatment goals and objectives for each child.

This program is just one more way the expertise of St. Francis Children's Center will impact the lives of children with special needs and their families.
